Arriving For an Appointment

When arriving for your appointment, it is necessary to check in at the registration desk, if applicable, a co-payment may be required.

Please arrive for your first appointment 30 to 45 minutes early to register as a new patient. Upon arrival, check in at the registration desk in the outpatient waiting area and indicate that you are a new patient. You will then be asked to complete patient information forms including insurance information and credit application. Payments for medical and surgical services are expected at the time of service. The medical center accepts several payment methods including: cash, check, Discover, MasterCard or Visa.
