Medical records are kept strictly confidential and will not be disclosed without your written permission. Patients requesting a copy of their medical records must sign an authorization form.

Birth Certificates
Copies of birth certificates can be obtained from the Vital Statistics Branch of the State where the child was born. For North Carolina, please send your request to:
